YOUR FIRST VISIT
PLEASE
READ THIS PAGE FULLY. Preparation for your visit is
essential in helping us to
see
you and all patients in a timely manner. Please
assist
us
by completing these tasks before you arrive.
1) Contact your insurance carrier and ask them to assign
Dr.
Alice Coghill as your Primary Care Physician. If you need a
Provider ID Number please contact our office at 212-244-4520.
2) In addition to contacting your insurance
company, please bring the following with you to your
appointment:
- Your
insurance card
- A
valid, government-issued photo-id card.
- Any
prescription medications you are currently taking.
- A
copy of your medical records from your previous Primary Care Physician.
- Please
understand your insurance policy and what it covers. For
example,
if you are coming in for a general physical please
verify that your policy covers this services. Many plans no longer cover the "annual check-up."
- Female
patients, please know the date of the first
day of your last menstrual period. This can help Dr. Coghill
determine appropriate tests and/or medication.
- Please
remember that all co-pays are due in cash or credit card only
before seeing the doctor. We do not accept checks.
- Please
go to the forms page (click
here)
and print out the Medical Information Release Authorization form.
Please complete a copy of the form for each of the other
doctors
which you are currently seeing, and one for your previous Primary Care
Physician. This will allow us to request your medical records
and
to discuss your medical history with others involved in your care.
3) Check your email. A few days before your first appointment, you should receive an email with the New Patient Registration Packet. Please fill out the forms completely. Bring the completed and signed forms to
your appointment and present them to the medical assistant at check-in.
If you are coming for a Pre-Travel Health Consultation, you should receive the Pre-Travel Registration forms instead. Please be sure to read our Privacy Policy.
4) About one day before your appointment, or immediately afterwards, please register for our ONLINE OFFICE
so that you may utilize our secure messaging capabilities to request lab results online. To learn more about our

6) Please review our FAQS page to familiarize yourself
with some of our polices.
Office visits are
for non-emergency conditions. If you
are experiencing
symptoms such as chest pain, shortness of breath or otherwise feel you
are experiencing a medical emergency, call 911 or go to the nearest emergency room.
