ABOUT TRAVEL VACCINATIONSIf you are traveling outside of the United States you should be sure to get the appropriate vaccinations. Whether you are visiting Africa, South America, Asia or even a nearby country such as Mexico or Jamaica, you should schedule a Pre-Travel Consultation to discuss recommended vaccinations and other health precautions. Just call 212-244-4520 to schedule an appointment or just click here. You should have your Pre-Travel Health Consultation 4 to 6 weeks before departure. Some vaccinations need to be taken in a series lasting a month or more. Please be aware that we do not file medical insurance claims for travel consultations and vaccinations in preparation for travel, so be prepared to pay for your visit and vaccinations at the time of your visit. All travel vaccinations and prescriptions require a consultation fee or $95. Your actual vaccinations are subject to an addittional charge. We accept Visa, Mastercard, Discover and American Express. We do not accept checks. All Travel Consultations are separate visits and cannot be combined with any other type of office visit. You should complete and print out your Travel Registration forms by clicking on the link below. Remember to bring them with you, along with your immunization records, to your appointment. Pre-Travel Health Consultation Registration Form PREPARE FOR YOURPRE-TRAVEL HEALTH CONSULTATIONTo gain the most benefit out of your Travel Consultation, please come fully prepared. You should bring the following: Your itinerary--Please include all countries which you plan to visit, and list them in the order in which you plan to visit them. Activities--Please be prepared to discuss your general plans in each location. Will you stay strictly on a resort, or do you plan to be more adventuresome? Will you be staying with friends or relatives? This can have a significant impact what precautions you should take. Immunization records--please bring your immunization records and/or proof of immunity (titers) if available. Your Primary Care Physician can supply you with this information. During your Consultation we will review the above information with you. We will provide you with advice on how to avoid illnesses and provide you with country-specific reference material concerning recent disease outbreaks, available medical care (if available) and other information concerning the country you are visiting. At the end of the consultation, you will be given your vaccinations and any special travel-related prescriptions needed.
