PREPARE FOR YOUR VISIT

Preparation for each visit is essential in helping us to see you and all patients in a timely manner.  Please review this list before each visit and complete each item.  We appreciate our assitance in helping us to help you maintain your health.

1) If you have changed insurance plans, changed address or changed your name please fill out a new Patient Registration Form and bring it with you to your appointment.  This is the first page of the patient registration material that you filled out as a new patient.  JUST CLICK HERE.

2) Always be knowledgeable concerning your medical insurance coverage.  For instance, if you plan on having  a physical, please verify that your insurance policy covers this services.

3) Female patients, please know  the date of the first day of your last menstrual period.  This can help Dr. Coghill determine appropriate tests and/or medication.

4) Remember, we only accept Cash or Credit Cards for payment of co-pays.  We do not accept checks.

Office visits are for non-emergency conditions. If you are experiencing symptoms such as chest pain, shortness of breath or otherwise feel you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
