Welcome to our online appointment scheduling page.  Before scheduling your appoinment, please carefully review this page.

1) If you are new to our office, please be sure to read our Privacy Policy. New patients will also receive a New Patient Registration Packet in their email.This packet will also contain some of our office and financial policie s.  Please fully complete the forms, sign them, and present the completed Packet to the receptionist at your first appointment. Failure to complete the form in advance could lead to the need to reschedule your appointment.

2) You will also recieve a confirmation email of your appointment. Always read this email carefully to insure that you have scheduled the type of appoinmtent that you wish.  If there is a discrepancy, please call us or reschedule your appointment.

3) Please try to arrive at least 10 -15 minutes early for your first appointment so that we can complete your chart and clarify any potential insurance issues.  After your first visit is complete, please register for our Online Office to request lab results, prescription renewals or a Virtual Office Visit (a.k.a. webVisit).

We are sure that you will  find this system very convenient.

4) Click here if you are scheduling an appointment for Travel Vaccinations.

• Comprehensive Preventive Exams are for "Annual Checkups" or Pre-Surgical Clearances.  All other visits (except travel vaccinations) are considered "Sick " visits.

• You will need your insurance ID # to schedule an appointment.

• All copays and outstanding balances are due before being admitted to see the doctor.

• Please complete a new Demographic Form if you have changed your address, insurance or name.

If you are ill and desire a "same day" appointment, but the schedule is full, please call us at 212-244-4520 and we will attempt to fit you in.

TRAVEL VACCINATION APPOINTMENTS

Please fill out your Pre-Travel Health Survey (which will be emailed to you), then print and bring the completed form to your appointment.  You may have to reschedule your appointment if you do not bring the completed form with you.  There are 3 types of Pre-Travel Health Consultations-- Basic, Intermediate and Comprehensive. You should carefully review what each includes and select the type of appoinment that you feel is best for you.   Click here to review the three types of travel vaccination appointments.

 It is the patient's responsibility to pay for all vaccines at the time of service.  We do not file insurance claims for these vaccines or for the consultation.

Please remember that we have regular (e.g., ill) patients scheduled before and after you.
