NEW PATIENTS
Before scheduling your appointment, new patients should review our office and financial policies. These are included in the Patient Registration Packet , which will be emailed to you before your appointment.
You must bring all of forms with you-- printed, completed and signed--to your appointment or you may have to reschedule your appointment. Please try to arrive at least 10 minutes early for your first appointment so that we can complete your chart and clarify any potential insurance issues. After your first visit is complete, you may use our Online Office to request lab results, prescription renewals or a Virtual Office Visit (a.k.a. webVisit). Please also review our Privacy Policy.
You will need your insurance ID # to schedule an appointment.
All copays are due before being admitted to see the doctor.

RETURNING PATIENTS
Please come prepared to pay, in full, your complete co-pay and any outstanding balance. Patients with outstanding balances will not be seen until their account is up-to-date.
If you have changed address or insurance coverage, please complete a new Demographics Form (just one page) and bring it with you to your appointment. If your last visit was before August 25, 2008 then please read, sign and date our Financial Statement and Office Policy.
If you are ill and desire a "same day" appointment, but the schedule is full, please call us a 212-244-4520 and we will attempt to fit you in.
PRE-TRAVEL HEALTH CONSULTATIONS
Please fill out your Pre-Travel Health Survey (which will be emailed to you), then print and bring the completed form to your appointment. You may have to reschedule your appointment if you do not bring the completed form with you. Pre-Travel Health Consultations are scheduled for 45 minutes to 1 hour. Please remember that we have regular (e.g., ill) patients scheduled before and after you. These consultations are $95.00 and are required to receive advice or vaccines or prescriptions for your trip. Vaccines are addtitional. It is the patient's responsibility to pay for all vaccines at the time of service. We do not file insurance claims for these vaccines or for the consultation.

Office visits are for non-emergency conditions. If you are experiencing symptoms such as chest pain, shortness of breath or otherwise feel you are experiencing a medical emergency, call 911 or go to the nearest emergency room. |
